I [name & surname] herewith grant Accor Services Poland Sp. z o.o., hereinafter referred to as “AUTHORISED USERS”, the express consent to the use free of charge of the photo, video depicting me and/or audio recordings with my voice made during the photo shooting/event/video shooting taking place during the P&C Community Event in Warsaw on 15-16 October 2026.

The aforementioned recordings may be used for the purposes of advertising, distribution and/or events of the AUTHORISED USERS in any form, in particular in the local press, on the internet, on social networks (e.g. Instagram, Facebook) and on intranet.

This consent also includes permission to mention my name. Furthermore, I agree that a text or comment may be attached to my image, my video and/or my audio recordings, provided that these are not derogatory, defamatory, abusive or generally pejorative or disdainful.

The use of the rights designated above may be also performed by ACCOR S.A. (82 Rue Henri Farman, 92445 Issy-les-Moulineaux, France) and/or one of its subsidiaries and/or a hotel operating company operating a hotel under an Accor brand.

I am aware that the photo, video and/or audio recordings of me can be accessed worldwide if published on the internet or in social networks. A further use and/or modification by third parties cannot be excluded.

The AUTHORISED USERS cannot guarantee that photos, video and/or audio recordings published on the internet are completely erased as third parties may, for example, have copied or modified the photos, videos and/or audio recordings. The AUTHORISED USERS cannot be held liable for the manner and form in which third parties use the images, video and audio recordings, e.g. downloading photos, videos and audio recordings and their subsequent use and modification.

If the consent is not revoked, it is valid for an unlimited period of time. It is also binding for my beneficiaries and heirs.

This declaration of consent shall be interpreted and exercised in accordance with European law with respect to the General Data Protection Regulation and otherwise in accordance with Polish law regulations. In the event of any dispute arising in connection with the performance of the obligations arising from this declaration of consent, the parties shall first seek an amicable solution.

I have been informed that this declaration of consent is voluntary. I have also been informed that I may revoke this consent at any time, provided that I grant the AUTHORISED USERS a reasonable period of time to remove the images, videos and/or audio recordings.

The revocation shall be sent by letter addressed to: Accor Services Poland Sp. z o.o., 59 Złota Street, 00-120 Warsaw, or by e-mail to data.privacy@accor.com.
